It's a proportion between two measurements on one face, not a comparison against an external template.
Why a single photo is the wrong tool for this specific measurement
Both distances in the ratio come from pixel positions in a 2D image, and a camera doesn't record a face at a fixed, distance-independent scale — it records a perspective projection. Hold a phone close and its wide effective field of view compresses features nearer the lens more than features farther away, which changes the eye-line-to-lip distance relative to interpupillary distance in a way that has nothing to do with actual bone structure. Step back and use a longer working distance and that same face measures differently again.
